A product link can preselect your interest; you can change it before submitting. The general-question form asks for name, email, question type and message.

Do not submit medical records, detailed health histories, government identifiers, payment information, passwords, or other sensitive information through these public forms.

Website requests and campaign labels

Website hosting and form services may process technical information needed to operate and secure these services. When present in a page link, campaign labels are included with a submitted form to identify the source of the inquiry. These pages do not send behavioral analytics events or use advertising pixels, third-party analytics scripts, cookies or browser storage.

Service Providers and Sharing

Website forms are processed through Google Apps Script. Submissions are stored in the website intake spreadsheet and sent as internal email notifications for follow-up. Website hosting, form and email providers process information to support these services.
